is a joint clinic run by the Doctor and Practice Nurse. During this
clinic routine Diabetic screening is carried out by appointment.
Objectives of Care:

Early diagnosis and management programmes

To provide appropriate education which will
enable Diabetics, where possible, to acquire the knowledge and
skills necessary to take responsibility for their own health
care and well being.

To maintain blood glucose within an acceptable
range.

To ensure patients with risk factors are appropriately
assessed and managed.
What will happen at my consultation?
- Discussion of diet and medication
- Recording of height and weight Blood
Pressure
- Urine monitoring
- Blood sugar monitoring
- Review by GP as appropriate Referral
to other services when appropriate:
e.g. Chiropody
Ophthalmology
Dietician
Follow up
